Documents required :
a) Duly accomplished Death Registration Form (Download Form here)
b) Original Passport with photocopy (deceased and applicant)
c) Original Death Certificate authenticated by the Department of Foreign Affairs (DFA) of the Philippines with photocopy
d) Police report (in case of unnatural death)
e) Embalmers Certificate with photocopy
f) Cremation Certificate with photocopy
g) Consent from next of kin for local burial, transportation of dead body/ashes
h) Booking Confirmation
